My experience as a lieutenant (US Army, 1973-77) squares with Fred's. As in when the brigade commander strolled into my office and asked why some of my trainees were up on a telephone pole splicing wires. "Top! Please fill in COL Sullivan on your latest."

Or a 2d lieutenant saying, "According to the map . . ."
